Note required form to be fill out prior to appt.
I really enjoyed working with Rhiana Holmes. She was very kind and understanding. However, I think it should be more clear at checkout you are REQUIRED to set up an account and fill out a form prior to the appointment or your appointment will be canceled and you will be charged a fee. I scheduled an appointment for a call, paid the fee and figured i would be called at the appointed time. However, in a follow up email i missed the we were required to fill out the form or we would have our appointment canceled. I was very confused when the appointment time came and went without a call. i then looked at my email, saw the request and filled out the form quickly one min into the appt, but was still rescheduled and a missed appointment fee payment was added to my account. customer service was very understanding and took care of the fee as a one time exception, but i was still frustrated by all the back/forth and cancelation of my appointment.
