The experience is actually confusing, i have to send duplicate forms to both you and Naurvaris. I do not want to hold up the approval and will do my best to get all forms submitted.

I get very good service throughout the year. However, the renewal process leaves a lot to be desired. I get the renewal application, fill out all the forms, make appropriate copies and wait for Dr. to sign and fill out the prescribed forms. This takes a couple of weeks. Then, I get a notice that there are missing forms. I call to find out that you haven't even received my packet! How does that happen?
