Barry B.
Poor information and instruction.
I submitted (in the envelope provided) my completed Prescription Lifeline application on 23 November feeling as though I had followed the rules as requested. On 23 December I received another application from Prescription Lifeline requesting that I complete a second application. I immediately called the office and explained my situation and was told that my application had not been received. I dashed to my physician's office to get his signature as he was leaving for holiday. I again mailed the application to Prescription Lifeline. On December 28 I received an e-mail stating that I needed a "Test Claim" from the pharmacy dated after 1 January 2021, showing the cost of a 30 day supply for the prescription (Apriso) from my pharmacy. I took the letter with me to pharmacy only to be told that I needed a doctors prescription before they could give me the "Test Claim" information. My doctor was on holiday and I did not get the prescription. The first week of January I contacted my doctor's office to get the prescription. The doctor's assistant is working from home and I was told that she would be in touch with Prescription Lifeline to satisfy your request. as off 10 January I have not received approval from Prescription Lifeline for my medication. I am fearful that I will be time barred from receiving my medication which I urgently need. My dissatisfaction with Prescription Lifeline is. The lack of timeliness in advising me of the need for a "Test Claim", which I could have received the prescription from my doctor when I was in his office. I have never had to get this before. Why was I not advised of the need for this form when I was sent the application.

Mr. Byars, Prescription Lifeline would like to apologize for the difficulty that you are having in renewing your application with the Bausch Health Patient Assistance Program. Unfortunately the Pharmaceutical companies are constantly increasing the difficulty in approval to their patient assistance programs. I am going to issue a 2 month credit to your account to allow time to get everything in order. I will have one of our advocates contact you to let you know exactly when your next draft will be. Thank you for allowing us to assist you with your medication, Prescription Lifeline.
