Each year the Pharmaceutical Company requires a new application for enrollment. Once the application is received and processed, your medication will start shipping out again. Sorry for any confusion. I had difficulty submitting the proper forms. I either did not understand or was misinformed the exact forms required. I think the drug supplier shares some of the blame. I spoke with three different representatives. All three were very courteous and professional. In the end I was approved.

Prescription Lifeline is good except their checklist does not actually ask for what they need for you to send in. I voiced my opinion a few times about this.
