I received a pre-treatment estimate from Delta Dental for the replacement of two crowns. The estimate indicated that my insurance would cover approximately $750. Based on that estimate, I had the procedure completed, and my dentist submitted the claim. After several weeks without receiving any explanation or payment, l contacted Delta Dental. I was informed that my claim had been denied. When I asked why, no one could provide an explanation. Since then, I have called customer service numerous times. Unfortunately, each representative has given me different and conflicting information. I have been told that: My claim was denied because my age was incorrectly listed as 12 years old. My insurance policy was no longer active. The claim was being resubmitted for review. Despite these conversations, no one has been able to tell me the actual reason my claim was denied. I believe I am entitled to a clear explanation of the denial, especially after receiving a pre-treatment estimate indicating coverage. 1 am exhausted from repeatedly calling customer service and receiving inconsistent information. I respectfully request that someone review my claim, explain the reason for the denial in writing, and let me know what, if anything, is needed to resolve this matter. Please help me resolve this issue as soon as possible.
