Patient Safety Violation / Medication Compromise
URGENT: Patient Safety Violation / Medication Compromise I am writing to you directly to bring an immediate, severe safety violation to your attention regarding your pharmacy operations. On 7/9/2026, I received a shipment of Cyanocobalamin (Vitamin B12) intended for my pet. The medication arrived boiling hot, and the included temperature safety indicator on the label was fully triggered, confirming a dangerous temperature excursion. When I contacted your pharmacy for a replacement, your pharmacist took me off the recorded line and actively instructed me to ignore the triggered safety label and administer the boiling hot medication to my pet anyway. This directive represents an egregious breach of pharmaceutical safety standards, consumer protection laws, and veterinary health ethics: * Destruction of Medication: Cyanocobalamin is highly heat-sensitive. Exposure to extreme heat breaks down the molecular structure, rendering the drug ineffective and leaving my pet completely untreated for a critical medical condition. * Contamination Risks: Severe heat causes liquids to expand, threatening the sterility of the vial's rubber stopper and risking bacterial contamination in an injectable drug. * Dismissal of Safety Protocols: Temperature indicators are placed in shipments specifically to protect patients. Advising a consumer to disregard a triggered safety mechanism is professional malpractice. As a customer, I am deeply alarmed and carry an absolute concern for the safety of animals and patients relying on your pharmacy. Had I blindly trusted your pharmacist’s advice, I could have severely harmed my pet by injecting a compromised, non-sterile solution. I demand the following immediate actions: 1. An internal investigation into this specific pharmacist's conduct and instructions. 2. A formal response outlining how your company plans to correct this systemic training failure. Please be advised that I am preserving the physical evidence, including photos of the triggered safety label, and am concurrently filing formal complaints with both the State Board of Pharmacy and the U.S. FDA Center for Veterinary Medicine. I expect a prompt response regarding this critical safety failure. Sincerely, Tara Sue
