Unfortunately, we were unable to cancel your order as you have already filled out the medical history/allergy form and signed the Attestation. The Attestation is a legal document that requires our customers to acknowledge and accept that their order is non-refundable moving forward. I have attached a copy of the Attestation for your review. Unfortunately, due to this, we are unable to cancel your order.
