Excellent service I received all paperwork that was needed for my dog for travel and housing. All of it was filled out by a Dr. I just have to bring the Medical/ mental health form to my Dr. To be filled out. Also the form for my Vet. To sign. I had called my Psychiatrist before going online to order this and he was going to sign a form for me because I am permanently Disabled. But I looked this company up and it it saved me a lot of time finding the forms I need. Because my Dr did not have the forms needed. I would differently recommend this service.

I was very please with the professional letters and documents received. I was able to print out my copys via email that allowed me to have our new rescue and ESA animal and give the letters without any problems or delay to the manager. I was skeptical about ordering the ESA certificate and paperwork online. My worry was unfounded as PDS center was very easy to deal with. Form needed to be filled out for the Doctor to review was easy to understand and with my explanation no follow up letter or phone call from the Dr. was required. The peace of mind and the reassurance of being able to take Abby with me is immeasurable. We are now able to go places that were off limits to us in the past and being retired now our destinations have become limitless. The conformation E-Mail and documents came in within 5 hrs of submitting the request. The original documents and the laminated card with Abbys photo and Dr.s name on it came in the mail in 10 days. The card is convenient and SO professional looking.
