Expect to pay A LOT out of pocket
Our small business chose Sana for its alleged flexibility and coverage, but it was a disappointing experience for me. Perhaps if all you only need preventative care, it's fine; however, I really struggled to get things covered outside of that.
For context, I was on the Executive team of a small Austin-based startup, working remotely from the Milwaukee area. It is worth noting that my experience may have been different were I in a more startup-friendly area where more companies used Sana.
As other reviewers have said, their network of doctors isn't as clearcut as a traditional insurer, and most of the places listed don't actually take Sana. For example, I went to an urgent care for a ruptured cyst – not only was this not covered initially, but to get Sana to cover it, I had to provide them with diagnostic codes (which you have to do for every single submission to them, btw). It's really difficult to acquire these – I had to submit a HIPAA request and wait for months to get them. Meanwhile, I'm still out $500 for this visit, which, btw, Sana would only comp me for once it was fully paid off (something to consider if you ever do a payment plan).
Visits to specialists were almost never covered, even if they were referred by my children's pediatrician and were allegedly in network.
I do want to give some credit to their support staff – Kelly in particular was amazing – but many of the others were dismissive and inattentive, asking me for paperwork that I had already provided in previous emails, often with a tone that wasn't particularly friendly.
In short, if you're a small business, I would probably opt for another option, even if the cost is a bit higher – your employees will thank you.

I am an employer
I am an employer. Everyone was super friendly and the coverage seemed to be pretty decent as far as paying for what it said it would, with a few exceptions. The invoices are a nightmare - they are just Excel worksheets; very unprofessional looking and not very helpful. The team is not on the same page about employer requirements, especially for very small companies. My contact person got pretty argumentative when I pointed out discrepancies between what she had to say vs. what the prior person had said. The portal also left a lot to be desired. I was overbilled several times for the accessory benefits (Beam, VSP) because the feed between Sana and Beam did not work. Overall pretty disappointing for the price. We had a huge rate hike our second year.

Not Right For Us
As a smaller mid-size business, the cost and additional perks that are packaged with Sana were great for our employees, but we began to see an increase in complaints about claims not being accepted and the network especially when a specialist was needed for treatment. The "all in-network" pitch didn't seem to be truly inclusive. When our renewal rate was significantly higher for 2023 we could no longer justify the headaches employees were experiencing with their medical insurance. After notifying Sana of our non-renewal decision it became almost impossible to get assistance when needed on the admin side, which was not a surprise because most insurance companies do the same, but it was disappointing to see Sana fall into that bucket.
