I set an appointment time, was online & waiting & no one ever came online or showed up. I waited online nearly 45 minutes. Very disappointed.

On 12/23, my son applied and paid for his medical evaluation. My son has seizures. Dr. Scott Dattel called on the 24th and told is he could help. On 12/26 the Dr. Dattel called again, stating he would help my son. On the 12/28 he emailed leafy doc and got a response stating he was certified. Still not received his physician certification. His state application is filled out and we are still waiting for his physician certification. On 12/29, after emailing again, he was told he should have it in 24 hrs. This is 1/4/2022 and he still has not received his physician certification. The last two emails he sent has not even been responded to. We live in Missouri. To this point he is extremely dissatisfied with the service he has received. He is disabled and is on a very limited budget and it seems he has been taken for 126 dollars. He and we are about to the point of contacting the Missouri Department of Health and Senior Services, if this problem is not resolved very soon. Update: Originally I gave leafy doc a rating of 1. After finally talking to them, they found that the physician had entered his ssn and birth date incorrectly, therefore the certification would not upload to the state application. The physician will correct this issue in the morning and hopefully this problem Wil be resolved.

This really was not a seamless service. I uploaded my documents to the NY state of health website 12/16 and still cannot access a temporary card. I paid for this service in good faith, very disappointing.

Waited 5 hours for physician’s call, after having been told he will call within an hour 3 times. Forms that were filled out at the time of scheduled call could have been completed long beforehand. Very disorganized business. One employee actually was rude and I believe was not truthful.
