I started with Form Health program 6 months ago and have lost 51 pounds to date. This program really works. I have an awesome team who I communicate with regularly, who provide lots of resources and who are always very supportive with zero judgement. I really like both my dietician and my doctor, they are up to date on the latest research regarding all aspects of weight loss and are genuinely helpful and kind. I would definitely recommend Form Health and would tell anyone who is considering joining to move forward

I have been on this program for approximately 5 weeks. So far it has been very helpful to me. I like the fact that there is accountability such as posting pictures of your food and sending your weight to them. I've had one appointment with the doctor and two with the dietitian which have both been very informative. So far I am down around 16 lb and even though I have a lot more to go I am feeling more confident that the changes I've made in my diet will remain.

I love this service. In particular, I really like scheduled monthly meetings with my Doctor and biweekly meetings with my registered dietitian. In between scheduled video calls, the clinical team keeps in touch via the app and comments on my progress and meals. Initially I thought taking a photo of each meal was a little goofy. I have learned to love it since it’s so easy and much better than entering all of my individual calories consumed with all of the other apps that I have used. Based on my lab results and clinical records, the clinical team put me on a regiment specifically designed for me. The results speak for themselves. In the last six weeks, I am down 13 pounds and will continue to lose on this program. I’ll give you another update in a few more weeks assuming that the app prompts me again.
