I really like all the providers at Form. They are very knowledgeable and accessible. I just don’t appreciate the fee of $149.00 per month on top of my out of pocket cost for the medication of $399.00 and the copay associated with seeing the provider. Since I don’t utilize their offerings to warrant that price tag I will be looking for other options in care.

The primary reason for FormHealth
The primary reason for FormHealth, in my opinion, is to provide prescriptions for Zepbound or Mounjaro to Lilly. There is not enough attention given to expediting this very thing: prescription submission. It is unacceptable to wait 72 hrs for an answer about a prescription when the drug — if prescribed — must arrive by delivery requiring another 48-72 hours. In the best case, it is a 6-day wait for a 28-day supply. That means that a refill request should be in process sooner than a week before; that is week 2.5 in a 4-week supply. I suffered a 10-day delay recently; I had to call Lilly Direct 5 times: they had no prescription; it hadn’t been approved; it was ‘in process’ — for 5 days?! I was leaving on vacation; my doctor knew this from weeks before. No Rx was submitted. I sent a text to the MD asking for it; that msg was sent immediately to one of your ‘agents’ — my doctor never responded — who unhelpfully gave me contact info for Photon. Of course they don’t write prescriptions, they had none; my doctor was on vacation. I had already discussed with her this 5-week period. The covering MD didn’t know my history, wrote for one 4-week supply. This started processing after another mandatory 72 hour review. I needed 2 refills to cover 5 weeks. I needed another prescription; another MD message text, another Photon referral; another LillyDirect call. In this one, a warning that I would not get another early fill for a full year: limited to 2 times in 365 days. This will not fit my life, and in my opinion is irrational & incomprehensible. Tirzepatide (Zepbound) is a maintenance medication similar to hypertension. It is not a Schedule II narcotic. I am a retired hospital pharmacist and I will not restrict travel plans to New Zealand in 5 months; to Japan 4 months after that. I will begin saving overfill in used vials as possible emergency back-up caused by FormHealth delays which will subject me to some level of risk in reuse. I will definitely seek out another more reliable provider so that I can travel and maintain my health. There will be many others just like me. FormHealth should properly address these issues: prioritize prompt prescription submission, knowledgeable people who can remedy obstructions and most of all facilitate; it would do well to rethink the basis of its existence, the purpose of these medications and align it with refill policies. Zepbound is not fentanyl. Educate personnel about what needing a prescription means. Photon does not write prescriptions. I was sent to them every single time I texted I needed one….
follow up:
I read your response for thoughtful action. I want you to also know this: that I am presently in France in the 2nd week of my 5 week vacation (after finally getting the 2 x 28-day refills I needed). And today — today(!!), ~2.5 weeks after receiving the last of my 2 refills in 365 days vacation rule — I got a text message from Lilly: that my next refill is ready for approval. What? I have 2 refills, 8 weeks worth on hand. Isn’t that somewhere in your records when I was questioned with dates, vials remaining on my 5th call to Lilly Direct?? That Lilly now has another 28-day refill immediately waiting for my approval when it just delivered a 56-day supply? If I were a drug addict, hallelujah! More drug to abuse. But this is a serious maintenance medication; I’m keeping track; I don’t need a refill. FormHealth needs serious attention here; why are you approving another refill?? This reflects sloppy, chaotic operations and frankly, makes the vacation rule look absolutely ludicrous. Is this what you’re encouraging? That If I were home, to get that next refill to squirrel away for my New Zealand or Japan trips? I can’t. I’m on vacation and you’re not paying any attention.

I appreciate the help I get from dietician and the dr but it would be so much better if I were with my family doctor, as I did prior.

I don’t like that I’m being forced by my insurance company to use Form Health. Due to a change in coverage, I can no longer use my trusted primary care physician for this type of care. They are forcing me to use this service otherwise I would’ve never joined. It’s some type of racket they have going with the insurance company to make them pay extra money for the same healthcare I was receiving from my primary care physician. That being said, the doctor here who is helping me is very nice, and I’m getting what I need but at the inconvenience of having to schedule additional appointments with a doctor I am being forced to use.

High monthly membership fee
It is more convenient and friendly than going to your doctor but you will pay a high monthly premium for that benefit. Their monthly costs are not very upfront. The wording leads you to believe it may be covered by insurance. That is not accurate, that is just referring to the doctor visits which are separate costs. The good part is the doctors can send your prescription to LillyDirect to help you save money if your insurance doesn’t cover the cost of Zepbound (and you are okay with syringe injections instead of the injector pen). The other services are pretty run of the mill offering you the same advice with a quick Google search.

Not Completely Happy
Same as before. I don't feel really seen or cared for my the doc or dietician. Mostly just feel like a number. The televisit with the doc allows me access to the meds but those are still very pricey.

Too much…
The amount of required visits with a dietician and Doctor do not serve any purpose except to get payment. Overall program is expensive and now that GLP-1’s are not completely covered, the entire program is expensive. Plenty of other online providers out there that won’t try to extract every last dollar.
