Form health has been a life changing program for me! It’s easy and the doctors and nutritionist are flexible and make the program easy!!

I was never into weight loss programs or extreme dieting, nor did I ever think I can change my lifelong eating habits. This program has changed my life and my health in so many ways. Not only in my eating habits, but also in how much better I feel. My primary care physician referred me to this program to help with a medical condition that I have. I didn’t think I’d be very successful. I have not only lost weight but I’m happier, my whole body feels better and my mental health has all been positively affected. When I had issues with getting prior authorizations from my insurance company, my team went above and beyond to make the proper phone calls and put in the time and work to help me get what I needed. I started out with very little faith in this program or any program and now I’ve made lifestyle changes. I never thought I could change my eating habits or the foods that I put in my body. This program is absolutely amazing, I believe in this program and and I will stay with Form Health. My Doctor and my nutritionist believed in me and now I believe in me on my journey not only to lose weight but to also nourish my body and become healthier. Not only did I lose weight, my stomach issues/digestion issues went away, my mental health got better, my physical health got better and with all that my life has improved. All I had to do was give it a try, it was is life changing and well worth that “try”.

The app is easy to use but the doctor and nutritionist monthly meetings are the key difference. They are so helpful and supportive and offer excellent suggestions and accountability. I feel like I have a team that cares. No robots!!

Form Health overall is a unique and savvy weight loss health provider. Form Health does not mimic a mosquito with constant pestering; I appreciate that. Guess I was looking for that all along. They are purposeful with contact and requests. As it turned out my own concierge internist admitted (9/22) to not being equipped to help me medically with a weight loss regimen; I was shocked at first given her credentials. She recommended an online-based weight loss provider, but I was deemed ineligible with that company due to age. I began my own search and found Form Health and liked what I saw and read. And I was eligible! I’ve been actively working with them since October and began Saxenda in early December. (In November I was mostly recovering from a surgery.) I have had occasional feelings of ‘greenness’ (pre-nausea) especially when titrating up. I am now at the ultimate, full dose (3.0). It helped as far as convenience to purchase a little 15L refrigerator that sits atop my dresser for storing the medication. I appreciate that the staff is savvy and interested in my experience and answers my questions. They offer many tips to help me. What I truly value about the staff is their professionalism and solid experience to draw from. I have lost 20 lbs so far. I am grateful for this reasonable, wholistic approach to weight loss and my external primary care doctor and associated doctors express pleasure with my results too. I love being able to wear some clothing that I haven’t tapped in quite awhile! The oddest thing for me is telling others about Form Health because we all have a similar vexing reaction to…the name, Form Health. (I can obviously live with it but will continue to pass this feedback along.)
