No not even close A week goes by you get ads. You ask for a call back. No one calls the. You find an appt is months out? I can do better with my primary care and a 20 office Visit. The value SHOULD be ASAP Script Done

FORM is an independent virtual obesity care program that provides high quality personalized and science-backed care for each of our patients. Each patient works 1:1 with a FORM Doctor and Registered Dietitian who create a personalized plan that addresses nutrition, physical activity, and mindset shifts. Plans may include a prescription for FDA-approved medication, if deemed appropriate. FORM is not an online pharmacy and we do not offer discounts on medications. A member of our team will be in touch to answer questions and to see how we can best assist you.

Waste of time. I signed up, spent ages filling out all the forms and signing loads of paperwork. Then a week later got a blunt message saying I wouldn't be scheduled with a doctor until at least March. I said this is unacceptable and wanted a refund and they said "great, see ya!". Don't waste your time!

You pay the $99 and then you don’t hear from anybody except to receive an email stating you won’t be seen by an MD until approximately March. I have sent messages for a week now and still no responses. This is very deceiving especially when at sign up you complete all they ask for and are told you would hear from a doc and dietitian within 24-48 hours. I want a refund and will take my business to a company that VALUES their customers enough to respond to them.

My nutritionist Emily is very proactive & encouraging, although I’m gaining weight because I can’t get access to my previous GLP-1 prescription. Since I’ve been off of it, I’ve gained nearly 10 pounds in 6 months :-(

Form Health is a wonderful team of Doctors, Registered Dietitians, Administrators and other staff. They are there to help you succeed in your weight loss journey. My journey is treating my food addiction with medication and counseling. My recommendation is to get involved now. I have lost approximately 70 pounds in less than a year. I am working on losing about 15 more pounds.
