Form Health has been great! I like being medically supervised. It took a few weeks for my first appt but no issues since then. I feel supported and get an immediate response to any question or concern. I was on Zepbound (cheapest through Lily Direct for me) but unfortunately the side effects were too harsh. I am on a hiatus from drugs at the moment but am getting the support I need particularly from my dietitian. My insurance picks up cost of virtual appts except for my copay. I have completely changed my lifestyle. This isn’t a diet for me but a new way of living. I have been able to stop one of my blood pressure medsand hope to also get off the other one too! If you are serious about losing the weight then I would highly recommend Form Health as a tool to use.

Honestly, I’m not sure why this service got great reviews. I’ve had nothing but trouble since I’ve signed up with them. Enrolling in itself was wonky, the staff seems OK when I’ve spoken to them, but trying to get my prescription has been a nightmare. I have been battling for two weeks to try to get my script refilled and all I get is run around from form health and from the insurance company. First, I’m told that the doctor on for health isn’t allowed to prescribe my medication then I get told that the insurance company hasn’t received all the necessary information saying that I’ve enrolled in the program. I really wish somebody would actually pick up a phone call from farm health because all of it’s done via the app which I think is very impersonal and not very good customer service friendly. Personally, I would rather not be going through this service but I am told I have to per my insurance company so I feel sorry for anybody that has to sign up for this company and deal with this BS.

I can only say great things about my experience over the last year. I came in with skepticism and to be honest, I was pretty agitated because my employer enforced a rule where we would only be able to use Form health to be able to get the weight loss shots. However, The attention to detail, the professionalism, and the progress that I’ve made has been definitely rewarding over this last year. I sat with a goal to just get to 200 pounds and honestly didn’t think I’d get there because of my impatient personality. However, I surpassed my goal and currently at 184 looking to lose another 9 pounds. I recommend that if you’re serious and you’re ready to make a move that is positive to make you feel better, look better and have overall health advantages to take this seriously and be ready for a transformation. Don’t take this and then skip doses because you wanna go on special trips, cruises, or other events because it’s not that type of diet neon this is a lifestyle change And if you take it seriously, you will see that it’s very rewarding. You don’t have to starve yourself, you can eat healthy and every now and then have an unhealthy snack or two and still be on track. Probably the easiest diet program or regimen. I’ve been on

I am very happy with my experience with Form Health. I am very satisfied with my assigned doctors and the Form Health Service representatives. I love the information that I receive concerning ways to improve my healthcare.

Same as before. I don't feel really seen or cared for my the doc or dietician. Mostly just feel like a number. The televisit with the doc allows me access to the meds but those are still very pricey.

The entire staff is very educated and excellent listeners. I feel as if I have a choice and control in this process vs tied up in too many rules / structure.

The staff are all universally great. Supportive, knowledgeable, and professional. There have been some minor administrative glitches along the way, but bottom line, they have helped me successfully access needed medication, modify habits, and lose significant weight.
