Form team is amazing and have helped me to lose and maintain the weight loss

I am amazed if all the hard work they have done for me even tho right I am on my journey they been there for me . My care team has gone above and beyond to keep me healthy, positive and determination. I don’t feel scared since day one they explain everything to me . I really appreciate health form.

The doctor and dietician really listened and offered wonderful suggestions to meet my stated goals. They feel more like a partner in my journey.

This program is comprehensive and set up for long term success. The NP, Dietician and the physician have been so helpful in navigating the weight loss process. The app is very user friendly, and full of great information. If I could give more stars, I would!

My experience with Form Health has been a positive one. Setting up virtual appointments is quick and easy. Meetings with my doctor and dietician have been professional, friendly, and informative. Customer Service has been responsive.

I haven’t had a Dr appointment in months. Seems like everything is set up for east coast residents. With the new Dr changes there’s never appts available at convenient times. Also all the classes are during the middle of the day. Am I the only person that works? Why can’t the classes be recorded so we can view at anytime? Not sure what I’m paying for when it appears everything is inconvenient
