Instead, their administrative delays and lack of communication forced me completely off my medication for over two weeks. ​Broken Promises on Transition Care: I was initially told by staff that they could provide a bridge dose for patients transferring mid-cycle to prevent treatment interruption. ​Bureaucratic Runaround: Rather than facilitating care, they hit me with endless roadblocks, automated messages, and rigid demands for records they claim can take 30 days to obtain. ​Unresponsive Support: When I explained the urgency and that their delays caused me to miss critical doses, I was left waiting for days with zero follow-up or actionable plan. ​A weight-loss clinic dealing with GLP-1 medications should understand the medical impact of abrupt cessation. I'm being coerced into using this service by my insurance provider. Prescription coverage for my GLP-1 medication is being dropped unless I comply with their demands to enroll in this service and provide them my entire medical history without any good justification for needing such broad and invasive information. You can find a lot more truth behind Form Health's true purpose if you read through their "learning center" for employers and providers (citation removed due to Trustpilot not allowing links in reviews). They clearly state that they will help employers reduce the number of GLP-1 prescriptions by pursuing "severity-aware prescribing." (citation removed due to Trustpilot not allowing links in reviews) The language they use to talk to their true customers (employers and other health insurance providers) is dismissive and hostile to patients. It treats patients as problems to be dealt with as cheaply as possible, and promises to cut employer GLP-1 drug costs without having to engage in writing policy that openly discriminates (they'll handle the dirty deeds quietly thought their in-house certified doctors who clearly don't have the patient's wellbeing as their primary goal). Furthermore, their privacy practices and policies are quite suspect. In order to register for the service, patients are required to sign a medical release that allows Form Health to re-disclose medical information with non-care-related third parties without notifying the patient and without legal recourse if they get caught (it states that legal privacy protections are also waived). I'll probably forgo signing up for this scam and self-pay for my medication, but I guess that's just more validation for them when my insurance provider sees the number of prescriptions they're on the hook for go down.
