I booked my appointment two days in…
I booked my appointment two days in advance an hour after my appointment you sent me a $10 off coupon to use still haven’t received the email to get my card. The doctor took three seconds to talk to me.

To clarify, the $10 promotion you referenced was sent after your appointment was already booked; however, we still went ahead and refunded the $10 as a courtesy. Regarding your card, please log into your Elevate account and download the provider recommendation. From there you can log into the state system and complete your application. In addition our support team is always happy to help walk you through everything and ensure nothing was missed. We also regret that your visit felt too brief. Our providers aim to be efficient while still giving patients the opportunity to ask questions, and we encourage anyone who feels they need additional clarification to reach out—your care and understanding are important to us. Great customer service
I always renew through Groupon and this time my redemption code didn't work. The customer service rep at Elevate fixed the issue for me immediately and set my appointment. These people are always very responsive, understanding and nice. Highly recommend.
