We reviewed your process and we noticed that you forgot to include the state fee in your package, which is a mandatory requirement demanded by the Health Department for the processing of your application. In the package that we provided you, this information was in one of the steps on your checklist. Unfortunately, we have no control over these Health Department requirements. You must attach the money order or state fee check to your previous package and you will be able to receive your certificate. This money is not for us, nor is it required by us, it is a mandatory processing fee required by the Health Department. However, we are sorry for the confusion and hope we can help you.
