I completely understand your concern and frustration, especially given your previous experience and the fact that you specifically reached out to confirm the formulation beforehand. At the time you were originally advised, the medication was not compounded with any additional ingredients. It appears this change happened around the same time the pharmacy was making formulation adjustments to remain compliant with newer FDA regulations and compounding guidance. I truly understand why this is upsetting, particularly since avoiding B12 was an important factor in your decision to move forward again. I would still strongly encourage scheduling a complimentary nurse follow-up call so they can review your concerns, discuss the formulation in more detail, and help determine the best next steps for your situation. Once shipped, the medication is packaged with insulated packaging and ice packs designed to help maintain product stability during transit. It is common for ice packs to arrive partially or fully thawed, especially during warmer weather, but the packaging is designed to protect the medication throughout the shipping process. We understand that your concern is not about general dose escalation, but rather that the physical amount of medication in the vial appeared significantly different from your previous order. Different pharmacies and formulations can result in vials appearing different in size, fill level, or concentration while still containing the prescribed total amount of medication. Because compounded medications may vary in concentration, the physical volume inside the vial does not always directly reflect the total milligrams dispensed. That said, we do understand why the visual difference raised concerns for you, especially given your prior experience with GLP-1 medications and injectable dosing.
