I am so glad I joined Form Health. My entire experience has been so positive and I would recommend this program to all of my friends that need metabolic reset. The support team is amazing. They help with every aspect of getting onboard to have a successful result. This is a safe and effective way to lose weight so I can live a more healthful and happy life.

This program really work talking to a doctor and nutritionist once a month help you alot if I have a problem I will text text them and will text me back

It would be nice to track physical activity in the app to and not solely focus on food intake

I have been working with the Form Health Team for 4 months. During this time, I have had a decrease of 10% in body weight. The program fits my lifestyle very well. With the help of the Form Health team i am eating better and making better choices when we eat out. Besides weight loss, lab results have improved as well. Enjoy working with the team. I have had a very positive experience with Form Health. This program and the support provided by my doctor and dietician combined with the medication prescribed - all three combined have been a WINNING trifecta for me.
