From the very first appointment, I have felt seen, heard and believed. I didn't get the push back that I have gotten at all my other doctors of try harder and do better. My doctor at FORM immediately validated my feelings and we had a productive conversation from that point on. I have loved the service, the nutritionist and doctors. It fits my busy life style and I'm finally seeing results!!

It’s just like every other program out there, no magic bullet. You eat less, eat more healthy food and exercise more. Even then, it’s hard to lose weight, so they’ll prescribe medication and that’s the thing that works. But your regular doctor can prescribe those meds too, so for me, it isn’t worth the expense.

I have been using Form for about 5 months now and I have really appreciated the experience. The highlight is the one on one time with a dietitian and a Dr. I have done other programs and they fell apart on that service and eventually just stopped. The support has been great. One item that needs improvement is the app but I manage.

Form is amazing!! They have the best team that has helped me lose weight while other programs I have tried hadn’t worked!! I would recommend them a million times over!!

I had an incredible experience with my membership with FORM. I was assigned to Dr. Joshi and Joanna as my dietitian. Both took a personal interest in my journey for weight loss. Dr. Joshi would order lab tests as needed. She discovered some abnormalities and kept a close watch on these results. She is very professional and personal in her approach to my case. I couldn't have been more pleased to be under her care. She even called me and emailed me when she thought there was an issue that needed to be addressed. Five-star rating for Dr. Joshi. Joanna, my dietician was very understanding and non-judgmental when I confessed I had slip-ups with my diet. After each session, she would send me Information pertinent to our discussion. Five-star review for Joanna. I had to leave Form due to circumstances not related to my care with Form. Dr. Joshi assured me that she would be available to help with my transition into another program. Overall, everyone at Form from Jessica in administration to my care team was exceptional. I would highly recommend Form to anyone who is considering a weight loss program. I was with FORM from November 2022 to April 2024
