My doc wanted to Rx zepbound to help with weight related medical issues. My ins co required me to be in a weight loss program for 3 months first. I knew I didn’t want weight watchers or noom so my internet hunt brought me to Form Health - it was easy to sign up, a scale that connects to them and the app was sent, it was a quick process to get appts, and #1 = my medical team is very knowledgeable and has been so helpful on my journey. Don’t get the wrong idea - it’s the basics - good food, portions, consistency, be active — but their support and Rx’d meds help soo much than being on your own. I’ve been on the program for 4 mo and have lost 13 lbs before getting the zepbound (beginning that med now after ins issue) A review much like this set me onto Form Health - hopefully my review does the same for another!

After speaking with my primary care physician in the Orlando Health network, I was disappointed to learn they don’t handle weight management and offered no helpful suggestions. While browsing YouTube, I came across Form Health—and I’m so glad I did. The sign-up process was incredibly easy, and they accept my insurance, which made everything more convenient. The nurse practitioner and dietitians have been amazing—super friendly and always available through the app, which allows for open and timely communication. I’ve been with Form Health for the past three months and have already lost about 35 pounds. It feels great to be supported by a team that genuinely cares about your progress and well-being. I highly recommend Form Health to anyone ready to take control of their health journey.

The app is nice and working well for me. Only thing that I can count against it is while you can see your medical team WHEREver you want you can’t see them WHENever you want. They’ve got bankers hours, which sucks for me or anyone working midnights or rotating shifts.

It took a short time to figure out the program but after I did, smooth sailing. I rate Form Health 5 stars - the two doctors I’ve worked with have been A+ taking the time to answer all my questions both via zoom & in the message portal. The dietitian is very good helping to address and help solve food & eating challenges. Customer Support is great as well - everyone I’ve encountered has been professional, knowledgeable and they really seem to take of their patients. Thank you, Form!
