I don’t like that I’m being forced by my insurance company to use Form Health. Due to a change in coverage, I can no longer use my trusted primary care physician for this type of care. They are forcing me to use this service otherwise I would’ve never joined. It’s some type of racket they have going with the insurance company to make them pay extra money for the same healthcare I was receiving from my primary care physician. That being said, the doctor here who is helping me is very nice, and I’m getting what I need but at the inconvenience of having to schedule additional appointments with a doctor I am being forced to use.

I have found my experience with Form Health to be very positive. The support that is given both medically and nutritionally has moved me towards a path of success.

This is a really helpful program where doctors dietitian and patient get to collaborate and come up with a realistic goal and plan. Both doctor and dietitian maintain open lines of communication, which is very helpful in keeping me engaged. Super format.

I have enjoyed using Form Health over the past year. I have seen great results with taking the Zepbound and also talking with the MD and dietician. I highly recommend using Form Health to help achieve your health goals.
