This is a horrible program, forced on me by employer health insurance, as a condition for continuing my medication. I have a relationship with my doctor. This thing is sucking up a lot of my time with intrusive questions not related to weight loss, automatically scheduled me for appointments with service providers at inconvenient times, spams me with texts on holiday weekends, is making me use their connected scale (the same model as the one I already own that my whole family uses, except, you know, already connected to their service so they want me to use a second one), and, is just overall not able to provide the same level of care as the team that I, you know, see in person. It's immoral that insurance overrides my relationship with my doctor this way, and the entire posture of this service is time wasting, intrusive, clumsy, and condescending. The best part is that the billing is completely opaque, so I have no idea how much I'm going to be charged for this "value added" service.

The team at Form Health put together a great program and provided the support to beat all the goals that I set.

My company is forcing me to use this virtual service of medical people who don't know me, instead of using my personally chosen, well-respected, expert endocrinologist. I would never use this service as a choice. They will respond now and say they want to learn more. But all you need to know is that I am being forced to use this company despite the fact that I had a perfectly great program already.

Both my physician and dietitian have been supportive and informative. The experience with them has exceeded what I was hoping for.

Dr Azharuddin has been the absolute best Doctor, advisor and sooo caring during the time I have been in the program. She has send me so much good information for me to read! She has been so caring with what I have been going thru during hard times while I was in the program. I looked forward to our appointments with her!!! I have reached our goal in weight and health!!! Truly thankful!!

I'm being billed for a service that was never provided, but I have to pay that bill to remain eligible for coverage. I've made multiple attempts to get this resolved, but there is no option to speak to a person... only email. I keep receiving the same cookie-cutter response that I'm responsible for co-pays. Nobody will even acknowledge the issue I'm raising.

I love the app. It's very helpful.

Horrible! I don’t need this oversight when my own doctor monitors me. It’s intrusive.
