Thank you for sharing your feedback. We completely understand how important it is to know all costs upfront — especially when you’re on a fixed income. Just to clarify, the $50 state fee is charged separately by the state, and it is required for all medical marijuana card applications, no matter which provider you use. Our website, booking pages, and confirmation emails all note that the state fee is not included in Elevate’s appointment cost, but I’m truly sorry if that wasn’t clear when you booked. Dr. was 11 minutes late for a 15 minute appointment. Did not answer all of my questions on the phone spent one minute on the phone with me and still did not submit the proper paperwork to the state. Then they said that they were having a problem finding me in the state and they were in Arkansas and not Arizona and then they asked me for my password to the state site which I thought was a red flag. I would never use this place again going to ask for my money back

To clarify a few points, your appointment with the provider was completed and you were approved. Occasionally, appointments may run a few minutes behind, as our providers give each patient the time and attention they need, just like an in-person medical visit. We understand that can be inconvenient, and we appreciate your patience when that happens. Regarding the state application: because you selected the Full Service option, we complete the Arizona state paperwork for you. The state requires us to log into the official state portal in order to do this, which is why we requested your state login. Without that, we are unable to assist with your application. We have reached out by phone and email to assist and also offered to refund the Full Service fee if you would prefer to complete the state application yourself. That offer still stands — just let us know which option works best for you. Our goal is always to support you through the process, and we’re here to help in whichever way you prefer.
