Coverage decisions about possible pre-existing conditions are based on the information available at the time of review (timing of symptoms/first treatment and the diagnoses recorded by the provider). An X-ray isn’t always required to make that determination; however, if additional clinical details (including imaging) indicate this was a new, acute issue, we’re happy to re-evaluate.
