Not designed for medical professionals!
I've been working with TherapyNotes as a PMHNP for several years now. The electronic prescribing integration has been extremely disappointing, with frequent lag, undependable refill request alerts, and what appears to be a complete lack of beta testing previous to update rollouts. If you're a therapist I'm sure it works great, but there's a reason it isn't called MedicalNotes. Also the telehealth integration is laggy, glitchy, and I'm still using Zoom despite our group paying for the telehealth add-on.

App is usless if you can't get on it
I have experienced a total lack of coordination between my desktop and the Therapy note App. The most frustrating part has been attempting to get this fixed. Customer service is completely unresponsive to the actual technical problem. Every time I reach out, I get the same unhelpful, scripted responses that ignore the specific sync errors I am reporting. I was provided "canned" advice than actually fixing a broken product. If you need a reliable EHR that functions across devices, look elsewhere. This app is broken, and the company doesn't seem to care enough to provide real human support to fix it.

Brenda, I obviously don't know why the app isn't working for you from this review, but I will have someone reach out. Our mobile app is brand new but we're not aware of any issues. In the meantime the web app is completely mobile-optimized. The mobile app doesn't do too much that the web app can't do, but the web app can do a lot more than mobile app.

This is a terrible EHR!
My psychiatry practice has been using Therapy Notes since 2020. It was chosen because it was felt to be most helpful for the therapists in our office, not the medical providers.
There have been continuous IT updates over the years. Perhaps some of these were useful for areas in which I am not involved, such as billing. But in terms of patient notes, the most critical component of any EHR, they are unnecessary, counterproductive and supremely annoying. I frequently say that either someone in IT is making changes to justify their employment, or they are giving needless assignments to IT interns as a teaching exercise. What they are not doing is focusing on how providers need to record and store information.
Here is the most recent example. Medications are the most critical data in any medical record. For years the Therapy Notes medication template showed "Prescribed by Others", followed by a separate section called "Existing Prescribed Medication" [prescribed by the provider].
This was followed by a section "Medication Moving Forward".
This was clear and easy to maintain. It worked great. You could quickly see any changes being made in the patient's regimen. And it was also easy to refer to the patient's other medications, if there was a concern about about a drug interaction or side effect.
After the most recent IT "update". All medications are now listed in alphabetic order, regardless who is prescribing them. Not only is it laborious to quickly review the meds I am prescribing, but this mishmash listing is a medical mistake waiting to happen.
To make things more difficult, each medication is followed by IT tabs "No Change", "Adjust Rx", and "Stop", regardless whether the medication was prescribed here or by another practice. This is absurdly, needlessly laborious.
To extricate myself from this debacle, I am now typing all medications, separated as before, into the Objective Content section of the note. To eliminate the recent IT change, for every drug listed I must now hit "Stop" and provide a "Stop Date" and "Reason for Stopping". My stated reason is "Bad EMR; now in Objective Content section." Needless to say, this takes a long time simply to restore my note to its original, usable condition.
Customer Service at Therapy Notes is very nice, but not really helpful in addressing complaints. When I complained about the Medication section problem detailed above, they passed along my complaint to someone in IT, who sent me an email saying "you'll like it when you get used to it." I won't. This was a change no one with any knowlege of medicine would have recommended. There was nothing that needed to change and Therapy Notes broke it.
Response to Therapy Notes reply:
We use MD Toolbox for our e-prescribing and have no intention of switching to Therapy Notes. MD Toolbox is reliable, easy to use, and unchanging. That's three things Therapy Notes is not. TrustPilot is filled with complaints about Therapy Notes e-prescribing feature. Why are you ruining your product just to get us to use your much-criticized e-prescribing? This is not good business and it is not good product design.
Response from TherapyNotes, LLCApr 14, 2026
We have been making numerous improvements to the experience for prescribers, with many more to come. The experience you are describing is for when you don't use our ePrescribe solution. Our ePrescribe feature will likely greatly simplify your workflow. We are going to have someone reach out to you to try to address your concerns.

Works until it doesn't
Works until it doesn't - then long wait time for service - typically they must consult with others - never get your problem solved.

We'd love to know who you are so we can get more information. This is not an accurate representation of our services.

Clinicians and Small Practices Beware
I have been a TherapyNotes user and paying customer for claims submission and clearinghouse services for four years, and my recent experience with a claims issue has been extremely disappointing. Given that this is not the first clearinghouse issue I have encountered with this platform, it raises broader concerns about reliability and accountability.
Due to a clearinghouse error affecting claims submitted through the TherapyNotes system, multiple claims were never received by the payer, resulting in over $2,000 in lost reimbursements for services I had already provided. By the time the issue was identified, the claims had fallen outside the payer’s timely filing window. What makes this particularly concerning is that these claims were marked as successfully sent within the system, creating the impression they had been properly transmitted when they had not. Because the issue was not identified in time, my ability to resolve it within the required timeframe was limited.
When the issue was finally identified, the responsibility for correcting it was placed entirely on me as the provider. I was required to reconstruct claims, navigate appeals, and manually fax submissions. While TherapyNotes provided a document to support faxing, the burden of correcting the error and attempting to recover payment remained with me, despite having paid for a service intended to handle claims transmission.
Communication around this issue was also inconsistent. We received an email in February 2026, but after speaking with another TherapyNotes user, I learned she did not receive any notification or the fax document, which raises concerns about how broadly and reliably this issue was communicated. There was also no initial offer to reimburse the claims that were never transmitted, leaving me paying for a service that was not performed. The ongoing runaround in trying to obtain reimbursement has been the most frustrating part of this process.
Support staff were initially courteous to my reimbursement request, but communication became increasingly vague and, at times, evasive. I have been working with both a support supervisor (Heather) and a representative from the sales team (Lymari), as those departments are not connected. This required managing communication across multiple channels and resulted in confusion and, at times, conflicting responses.
After multiple exchanges, I was issued an $11 credit, but I am still seeking reimbursement for the July 2025 subscription fee of $78.37. I was told that full monthly reimbursements are not typically provided for claims-related issues, but this is not a typical issue. The system failed to transmit claims that were marked as sent, the error was not identified in time, and I was required to manually complete the work the service was intended to perform.
As of March 20, 2026, the request has been re-escalated with no resolution. At this point, the continued back-and-forth over a clear service failure, without acknowledgment of responsibility, is unreasonable.
For clinicians running small practices, failures like this have real financial consequences. A system designed for billing and claims submission should not create this level of risk or shift the burden onto providers to repeatedly follow up and correct errors they did not cause. Providers should also be reimbursed without needing to remain in prolonged back-and-forth to resolve a clear service failure.
Update March 21, 2026: Therapynotes asked me to update my review...so here is the update...
After continued follow up, I was fully reimbursed for both the claim submission fees and the July subscription fee.
For clinicians in Massachusetts who were impacted by this, I strongly encourage you to contact TherapyNotes and request a full refund for any claims that were marked as sent but never transmitted, as well as the July service fee. This appears to have specifically affected BCBS MA claims submitted during the week of July 4.
I was reimbursed for the system failure, and you should be too. I recommend contacting the Sales team directly.

Therapynotes is progressively becoming…
Therapynotes is progressively becoming more interested in making money than in creating an adaptive and functional product. For example, they have strongly pushed their AI and upgraded web features, both which cost inordinately higher amounts of money. Yet, despite many requests to make a simple revision to their video session history, so a clinician can know when the session started, when it ended, and total amount of time (like done on Psychology Today and other secure video applications). Ultimately this puts therapist at risk for risk to their license and insurance billing by inadvertently over billing. When requests are made it is always the same "we will pass it along for review". When a request is made to talk to a management person this is always flatly rejected.
As a response. My feedback has been submitted numerous times literally years. In this time Therapynotes has made substantial improvements and customer service is excellent. My long-term complaint is the management has no direct interactions with its customers, this fix is simple (i.,e Psychology Today had it as a feature when it used telehealth at the start of Covid), and it places therapists at jeopardy by not being able to effectively document the actual time of service.

Your feature request was reasonable, and it is in the queue for consideration. If your expectations after you made this request was that we would immediately release it, that is not realistic. We have a big queue of work to do, and updates take time. If you feel our services are only worthy of a 1-star review, we encourage you to find a service that better fits your needs.

Customer Service Failure
I have been with Therapy notes for a few months and find their support to be terrible. I have been trying to set up my EPCS for weeks and you can never get through to EPCS help. I am going to start looking for a new EHR today. If you are thinking about Therapy Notes as a nurse practitioner for your EHR, DON'T. I did get through to regular support and they put me to the e-prescribing queue. Was Caller "0" and stayed on hold for 45 minutes before giving up. That has been my experience every time I have called for EPCS. At least the regular support team answers but they can't do anything with prescribing

Our call queue was especially busy during the holidays. High call volume plus a lot of people out for the holidays. The phrasing "caller 0" was an issue with the phone system that we have resolved. It just meant you were next.

Try another EHR
Try another EHR, do not get locked into this as you will be penalized when leaving. You will understand if you pick this garbage company to deal with.

Your account went into storage mode in 2023. You called to reactivate the account and although you dispute this we sent you a call recording as evidence. We have offered to waive your accrued charges, but have simply asked you to log in to cancel your account. Our Success team asked you do this because this will purge the medical records on your account. However we've reviewed your account and have determined it's ok to purge because it's been delinquent and inactive for so long. Have a lovely day.
