Each request is reviewed by a licensed physician and, when appropriate, issued as a digitally signed, verifiable document. I was worried at first, but the experience was great!
I was worried at first that my job wouldn't accept the note, but they did! I am very grateful cause I wasn't feel well at all. The process was really easy and the form is quick. Also the note got to me in less than 2 hours! The ease and how cheap it was will make me come back again if I need a note.

Amazing e
Amazing everything went well and didn't question it I had very serious sickness I couldn't use teledoc I was incapable of using the phone or leaving my house

The notes I’ve been given have always been accepted for Amazon DLS! I’m really thankful to have found a doctor who actually fills out the forms correctly- taking away the anxiety and stress of it getting denied.

Fast service. The language used made it seem like this note could have come from an online source: "based on information presented" and "no further documentation is needed."

SickSlip is a telehealth practice, so our notes do come from an online visit, but every one is reviewed and signed by a licensed physician, carries the physician's NPI and state license, and has a document ID employers can verify at sickslip.co/verify.
