We'll be reviewing our website to ensure both costs are clearly presented upfront. Regarding the state application fees, these are separate charges levied directly by the state of Oklahoma. Hi Marylou, we thank you for leaving us this comment. We certainly take our process seriously, and no patient that is ineligible for their card will ever be charged in our company. Whenever this does happen by mistake, an immediate refund is issued. In your case, we see you were billed on 03/04, and your refund processed 03/05.
