Theresa, we apologize for the inconveniences. We do see that the doctor reviewed your registration with the state a few times, but for some reason it wasn't showing for you online. It might have been an incorrect number we had registered for your SSN. However, upon your request and because of this issue, we processed a refund for you on April 27th. Hi Amanda, as mentioned to you via phone call, in order for you to fix the issue, you need to contact the state directly; the state will not answer to anyone other than the patient. Hi Sue. We apologize for the inconvenience at the time of your appointment and the delay the doctor had that day. After your evaluation, the doctor was not able to certify you, as your card is still active. The system would have allowed the doctor to complete your certification process after 3/12/22, however, based on your request over the phone with our customer service representative, we have processed your refund today.
