Hello Cassie, we apologize for the delay with the doctor's call to get your evaluation done, and we're glad you were able to get your evaluation done later that day. Regarding the SSN, doctors require this information in your state to be able to generate your electronic certificate in the state website. After reviewing your file, we see that the doctor issued your certificate that very same day, and sent you the instructions for you to complete the online registration with the state. Once you complete this step, the state will issue and send the card to you in the mail. If you are having trouble registering with the state, please reach out to us at 833-245-3239 and we'll gladly assist you. We make sure that all patients are able to complete their registration and get their cards. Thank you!
