FORM is an independent virtual obesity care program that provides high quality personalized and science-backed care for each of our patients. Each patient works 1:1 with a FORM Doctor and Registered Dietitian who create a personalized plan that addresses nutrition, physical activity, and mindset shifts. Plans may include a prescription for FDA-approved medication, if deemed appropriate. FORM is not an online pharmacy and we do not offer discounts on medications.
