I started this plan a month ago. I really appreciate all the support from my doctor and dietician. Setting up personal goals is not my strength, but with their help I have been able to set weekly doable goals. I am down 6 pounds by being mindful, working on my weekly goals, uploading food pictures and having support and accountability from everyone at Form Heath. Thank you!

I have been very happy with the staff and program.

I’m new to the program. I love the interaction with both the doctor and dietician. The app is simple which I like. Take pics and upload. The best part is all the literature they upload to educate on effective weight loss/ maintaining weight/ working towards goals. The first couple weeks I was intimidated or even forgot to take pictures of my meals and uploading. They suggest uploading my journal. I’m now doing both. It really keeps me aware of selections.

Form Health is wonderful! I am working with a fantastic dietician who is helping me get away from bad habits and what is the best plan for me. My Doctor is terrific too! She only wants the best for me and it shows! Having one-on-one time to discuss all my issues is helping so much. I’m looking forward to my next session with her.

For the first time in years I have hope. I have been in every type of weight management program I can think of. I even had a lap band. I used the stress and domestic violence in my life as a reason to give up on losing weight. What changed for me was a program that understood the struggle of being obese and not being able to will it away. It has always been my fault my lack of will power, my laziness that kept me obese . Finally I learned through the staff of Form Health that I am human, obesity is a disease that laziness and lack of will power is not the reason for my lack of success.Dr. Chowdhury was the first medical professional that showed me respect and empathy. Form Health has absolutely changed my life! In 3 months I’ve lost 24lbs but more importantly, I’ve learned so much from my dietician about healthy nutrition choices. My medical doctor really helped me understand the genetics involved in my condition, which helped me take control of my health. I meet with my nutritionist every two weeks - the accountability for me is the biggest factor in my ability to stick with this program. Everything I’m learning I know I’ll be able to continue to make use of moving forward. If you’re considering the program and are concerned about the monthly fee, know that it’s well worth it for all the support the program offers. If you’re like me and feel like there’s just no hope to get healthy again, I HIGHLY encourage you to give it a shot. No contracts, you can cancel any time.
