Dr Baptiste and Amanda have been very helpful in getting me on track.On first meeting them they both gave great insight on my health in eatting habits and made sure I was included in my plan. I am sure we will make a great team.

I have tried other programs or strict calorie counting diets before. Always resulting in feeling hungry/fighting to keep the weight off. Eventually giving up and regaining (plus some) weight. So far, I’ve lost 24 pounds on the program. More importantly, I feel very well supported by my dietician and doctor. The approach of the program feels more sustainable and is teaching me healthy behaviors toward food that I’ve never been able to master. Highly recommend Form!

I am so grateful to my Form Health doctor, Dr. Simpson, and dietitian because they helped me get to a weight where I could get pregnant and now I've become a mom for the second time! My doctor prescribed a medication that was a game changer - it's like a switch flipped and I actually felt satisfied after meals. My dietitian Julia is the most amazing person ever - she's smart, encouraging and helped me change the behaviors associated with my eating patterns. It took the pressure and stress out of eating healthy and made me feel like this was something I could do. Losing weight is still hard work, but it's possible with the support of weight loss experts.
