I receive excellent care from both my physician and dietician on a consistent basis. It’s extremely convenient to be able to conduct my monthly health checks with them virtually and remain in contact with them via text. Uploading images of my weight and food diary on the app is simple and a helpful tracking tool. The medication I am prescribed has truly turned my life around and I’ve lost 42 lbs over the course of 11 months - steadily and healthfully. I receive many very helpful resources on nutrition, exercise, mindfulness, etc. from my dietician. Overall, Form Health has been fantastic in providing great care during my weight loss journey.

Form is the real-deal for weight management. Each member has meetings about every three to four weeks with their physician who specializes in weight management and their registered dietician. Members have a personalized treatment plan and goals to meet between appointments. If weight loss medications are indicated, the doctor supervises and monitors the member closely. This is not a quick fix program. It is based on solid evidence-based science and results in slow, healthy, long-lasting weight loss.
