I could not be happier with the NP or the dietician I am working with. They are both kind, knowledgeable, and supportive. I’d like to give an extra special shoutout to Victoria Butur who has gone above and beyond with the Zepbound shortages to call in scripts at any pharmacy I can find to get the dose I need.

Dr lowden and the form team are absolutely amazing! I have lost 60lbs in almost 2 years thanks to their support and guidance. I cannot imagine how I would have made it through without this team of incredible people!

Having done literally every program out there (seriously—name one! Done it) over the last 30+ years, I’m thrilled to have finally found something that works AND is sustainable. My favorite thing is the support I feel, from the admin staff, to the medical team and the nutritional support. I’ve never felt so confident that something is going to last and not just be a short term fix. I’ve been a FORM member for 9 months, which is honestly the longest I’ve stuck with anything. That’s because it’s easy! Sure, I have to put in the effort, but they make it feel easy. I still wake up every day and can’t believe it’s real.

What has made my experiences with FORM has become an essential part of my lifestyle when I made the decision in January 2024 because I have finally met a Dr who truly cares and understands my obesity concerns. I have also met a great dietician who gives me advice on what foods are healthy choices. This has been the best decision that I feel has finally directed me into a better lifestyle and succeed in my weight loss journey!
