They could not submit convincing prior authorization paperwork for my insurance to cover my Zepbound 4 months ago.

Horrible experience, I signed up and paid the fee and uploaded my drivers license and insurance and got no where. I woke up to a message saying my membership was terminated. No reason given. Never got any appointments or medications. I think it’s unnecessary to have a grown adult be forced to see a PCP just to get weight loss medication. I never go to the doctor and avoid doctors appointments because it takes most of the day sitting in a waiting room then paying someone to treat me like poop. No thanks!!! They stated that you absolutely had to be “under a doctors care” like Im an adult I don’t need a doctors approval to exist an d make decisions for what I want to do.

Greatly disappointed. I have been trying to find an all in one holistic care with everything that I would need in one place. Will the only thing that I’ve gotten in my money taken. No first appointment and the same message being sent about them trying to wait to hear from my Dr. when I sent them what they wanted and my primary stated that she hasn’t received anything. My think how do you take $149 with NO service provided. I sent an email stating that they could just cancel my membership and send my money back. And what happened NOTHING. No message no email NOTHING from them. Don’t do it. Not worth the headache.

Bait and switch. They say they bill insurance but they don't and they refuse to submit, instead they want you to pay a monthly membership fee plus doctor visits. Insurance confirmed they never submitted any claims despite the billing department saying they did submit and got denied. Edit to add: You can't get a reply from anyone beyond a generic this is how it is response. My husband has emailed and I sent an email. There wasn't even an attempt from Form to transition his care as stated in their Faqs. Just a solid you're cut off if you don't give us a monthly membership fee and pay for doctors visits. Again, nothing was billed to our insurance yet they claim it was all denied. I stand by the bait and switch allegations.

I started with Form Health in September for weight loss medication, hoping for supportive, individualized care. Unfortunately, my experience has been anything but that. They consistently treated me like just another chart. Sessions are already limited to just 20 minutes, and yet my provider was often over 5 minutes late—with no acknowledgment. The time I did get felt rushed and impersonal. The most upsetting part was when they threatened to reduce my medication based on an arbitrary “ideal weight loss” number—one that was never discussed with me. I followed their plan exactly. My progress was healthy and aligned with my goals, yet instead of support, I was penalized. The dietitian experience added nothing meaningful—just generic PDF documents and more food stress. It felt like a checkbox service, not true support. To top it off, their so-called “team” has repeatedly failed to coordinate basics—like whether I need bloodwork again or not. The communication gaps are exhausting and unprofessional. As a woman and a patient, I expected to be treated with care and respect. Instead, I was invalidated at every turn. I would not recommend Form Health to anyone looking for compassionate, reliable care. You deserve better.

I haven’t had a Dr appointment in months. Seems like everything is set up for east coast residents. With the new Dr changes there’s never appts available at convenient times. Also all the classes are during the middle of the day. Am I the only person that works? Why can’t the classes be recorded so we can view at anytime? Not sure what I’m paying for when it appears everything is inconvenient

Honestly, I’m not sure why this service got great reviews. I’ve had nothing but trouble since I’ve signed up with them. Enrolling in itself was wonky, the staff seems OK when I’ve spoken to them, but trying to get my prescription has been a nightmare. I have been battling for two weeks to try to get my script refilled and all I get is run around from form health and from the insurance company. First, I’m told that the doctor on for health isn’t allowed to prescribe my medication then I get told that the insurance company hasn’t received all the necessary information saying that I’ve enrolled in the program. I really wish somebody would actually pick up a phone call from farm health because all of it’s done via the app which I think is very impersonal and not very good customer service friendly. Personally, I would rather not be going through this service but I am told I have to per my insurance company so I feel sorry for anybody that has to sign up for this company and deal with this BS.
