Form experience
The people that I talk to, nutritionist, Dr. and Nurse Practitioner are all great, very helpful, Understanding. The only reason for my 4 starts was on a recent change on medication, it took over a month to get the new medication to my pharmacy and I ran out of my previous medication. Otherwise everything has been great with Form.

Unfortunately, medication coverage and cost depend on your insurance plan an supply chain and stock difficulties may sometimes cause delays at your local pharmacy. If a specific medication is not covered or changes are made to your coverage, your Form Health physician will work with you to develop a plan that takes your unique needs and insurance plan into account. The doctors and nutritionist are fantastic.
The doctors are very friendly and have gone out of their way to work with me. The nutritionist has been great with providing me everything I need to succeed with my crazy schedule.

I liked being introduced to Zepbound
I liked being introduced to Zepbound. I also appreciate all the advice and tips on how to not only lose the weight but to keep it down. I’m trying to make a lifestyle change as I enter into my retirement years and I’m really hoping to live a more active and healthy life when I get there.

I appreciate the access.
I appreciate the access to a dietitian and a medical professional. My PCP would just prescribe meds with no consultation or support. It is reassuring that if a complication should arise, I have quick easy access to an experienced professional.

Great support team
I absolutely love my care team! I enjoy the classes, videos on social media and support group. The reason I didn’t give a 5 star review was because the billing is confusing and not self explanatory. When you try to reach billing by phone you can not get through. I emailed billing and it was basic answers. Not helpful.

The positive support from the Form Team.
The support team with Form is great. From the initial setup person to my personal team of physician, physician assistant, nutritionist, and general support has been great. Always professional, kind, and understanding of my personal health and weight challenges. They do not make me feel like just a patient of Form. Instead, I feel like this program is specialized to help me be successful in reaching my goals in the healthiest way for me.
