I feel the doctor and nutritionist are really good at making you feel like a person and not just a number/patient. I feel this could be difficult being online. While doing this they are still able to give good advice and sound medical information.

The staff are all universally great. Supportive, knowledgeable, and professional. There have been some minor administrative glitches along the way, but bottom line, they have helped me successfully access needed medication, modify habits, and lose significant weight.

I have been a part of Form Health for a couple of months now. And I do enjoy the collaboration between the doctor and my dietitian. They help me stay on track with my eating, exercise and water intake. I’ve noticed some changes in my weight & energy level. I know I can do better but most of the time I’m given ways to help in those areas.

The app is nice and working well for me. Only thing that I can count against it is while you can see your medical team WHEREver you want you can’t see them WHENever you want. They’ve got bankers hours, which sucks for me or anyone working midnights or rotating shifts.
