I really prefer meeting in person in an office. I am not a virtual health person. The Dr is very polite and knowledgeable, I just dislike the process.

The staff are wonderful. I haven’t found a need for the support person given the providers are all so helpful and thorough. The scale is large. It’s hard to have out in my very small house. Form sends too many notifications for upcoming appointments. For one appointment you get a reminder from the app, the support person, an email and you have to go in to confirm on top of all of the notifications. That’s 8 times I have to go in to address an appointment on the weeks I see the nutritionist and provider.

I have been a part of FormHealth since January 2025. It is part of my employer paid health insurance since I am taking a GLP1 medication. I like meeting with my team which consists of a dietician, nurse practitioner and MD. I have the app which is mediocre at best. it is very simple. The scale you get when you sign up from the program transfers your weights into the app which is nice but the problem is, the app does not allow you to look back at your previous weights. It only supplies you with your last weight. It would be nice to be able to look back at all of your weights that have been entered. Other than using the app for weight tracking, I do not use it otherwise.

Once connected with a doctor and dietitian it has been easier to set up appointments. I wish the app had a feature to determine how many calories I am eating. I have been maintaining now for several weeks and really want to make more progress towards my goal. I also don’t want to double track in another app.

Having to change provider mid treatment is not a good experience after relationship and history has been established.

Took months to get a medical provider available and assigned. The dietitian has still not been billed to my insurance, and I am concerned about that. The last two visits with my medical provider have been very rushed.

Be prepared to wait for an appointment. It took forever for them to get my medical info and schedule first appointment. First appointment was scheduled for 3 weeks after they got them medical info. Then a few days before my first appointment they moved it out another 3 week. Now, I a week prior to my appointment I have to change the appt for an important work meeting and earliest option is a month after! I signed up end of July and have yet to talk to a person.
