The FMLA form came thru. There is the issue of the $49 being a reoccurring fee to be charged. Check your account. I see that when I log in to view the charge and click into it. It was never discussed or authorized to be a recurring charge. I’m going to send an email today to specify that I don’t agree to that. My bank said if they charge again I have the email as proof that I didn’t consent. Other than that the customer service could be better. The only way to communicate is via email. It’s a little frustrating that there isn’t a number or chat but overall I got what I paid for

I contacted FMLA MD who was in partnership with Hinds Family Care at the time. The provider FMLA MD's Dr. Leah quit the partnership as I was trying desperately to have papwork filled out properly. My employer's FMLA forms all of them needed to be filled out and correctly at that. This was so stressful because the back and forth chats with colleagues every day to have Dr. Hines fill out all the forms needed for short-term disability and the colleagues saying that it was faxed over properly to have my appeal DENIED. I suggest that you make a list if questions to ask and a list of what you need done. Including ALL forms needing filled out. Ask questions and be specific.
