I received a YourRewardCard in the mail. Had $25 on it, no reloadable. Says "funds never expire" . Has an expiration date 02/30. Back says "12 months after activation will charge $2.50 month fee. Did not activate the card or set a pin. Front said $25. Not valid for cash or ATMs. Not valid outside the US. I believed it to be from my health insurance company, UHC. Can't be used for just anything, similar to the UHC U Card. Used it online at Amazon. Haven't tried to check balance. Should have little over $14 left on it.
