New debit card, cancelled after 2 years.
My bank replaced my debit card- after paying for 2 years they cancelled me with no way to reapply. Waste of money. Use anyone but them.

I wish I had known about this company before I signed up. This dental company is a scam. I signed up under their federal plan as I am a federal employee but stuck paying a full bill. Dentist took X-rays on two teeth that needed crowns. Both teeth are fractured and one mostly gone. Dentist even wrote the explanation along with X-rays submitted and claim got denied even after appeal. How you deny someone crowns that has part of their tooth missing. This is a horrible dental company. I’ll be switching when open season comes. Do your research on this company and you will see many bad reviews.

Terrible, terrible company! My Medicare Part C plan, Blue Cross Keystone 65, recently began using Dominion National for their dental coverage. Dominion is not paying my dentist! He even sent them my dental X-rays showing the teeth he treated. *I* finally was told that he had failed to file a "narrative" to justify why I needed a crown, which was a covered item. This came after he had begged them what else they wanted from him; they never once told him about this "narrative". Robbers! Liars! They farm out complaints to some company in India, whose responses are scripted and difficult to understand.

If could give a zero I would. They are unable to provide me with EOB reports for services provided and paid for. Terrible provider network. Majority of site are public assistance clinics. Spent HOURS searching for an in network dentist. Unfortunately this is bundled with my medical plan which is otherwise very good.

Terrible dental insurance provider. I have tried for weeks to find a dentist's office that accepts Dominion National. I finally got a new job with a different insurance provider. Please avoid Dominion by all means possible!

The UHC of dental care. Purchased Choice PPO plan over a year ago. Plan promised to cover fillings 50%, they paid $33 on a $268 filling, due to a loophole that said it only covers cheaper versions of care. In-network dentist they paid out as if out of network. We asked for pre-approval, they waited the maximum number of weeks and then denied pre-approval. Refused to talk with me after I pointed out errors, told me to submit an appeal and wait 180 days for them to process. Altogether we've gotten over $4000 of dollars of work done with in-network dentists that should have been covered 25% to 100% since we're in the second year of service. They've paid out a whopping $262.46 and left us on the hook for thousands.

I experienced numerous technical issues with logging into their website due to some issue with my prior membership with Kaiser Permanente. That one took months to resolve. Another technical issue is related to their autopay system. I enrolled in their autopay system which failed to automatically adjust my monthly premium payments when Dominion raised their rates from one year to the next. Now I've been on hold for 45 minutes waiting to speak with a customer service representative to resolve this issue.
