Thank you for taking the time to leave a review. The physician is licensed in Oklahoma and the certification is valid. The physician's mailing address is out of state but this is very common with telemedicine appointments. As long as the physician is licensed in Oklahoma and registered with the Oklahoma Medical Marijuana Authority (OMMA) then the certifications are valid. I will have the doctor's assistant reach out to you today to walk you through the steps of submitting the certificate to OMMA so you can receive your card. If you would like to speak with the physician again he can give you a call.

Hello, thank you for taking the time to leave a review. I have reviewed your paperwork and everything appears to be in order. The physicians signature is there at the bottom of the document in the Physician Information section. The doctor did not specify the medical condition because it is an optional section and most patient do not want their medical conditions listed for privacy reasons. The first physician signature halfway down the page (Certification of Necessity of a Caregiver section) is blank because that is only signed if a caregiver is required. If you have any questions or require additional assistance please reply to the email that had the medical cannabis certificate attached or call our office at 804-893-5068. If something is incorrect or you needed a caregiver please let us know.

Thank you for letting us know. I will have our office administrator reach out to assist you ASAP. Occasionally the State portal does not work properly which is why we include a copy of the digital ID card in the email with the after appointment instructions. We can have the provider resubmit the certificate if necessary that usually resolves any issues.

Thank you for taking the time to leave a review. The Cannabis Control Authority (CCA) pushed a new update to their online portal which is the state's online website where providers enter patient information. This new update prevents providers from coping and pasting some information when competing the medical card document which means they must hand type the Date of Birth, Email, and Driver's license field. This update went live yesterday and we are working with our providers to implement a strategy to prevent frustrations like this in the future. In your case it would be to assume that if none of the other characters in the email are capital letters then it is safe to assume the middle character after the dot (.) is not a capital letter either. I do apologize for the inconvenience this caused you and I am grateful that you have brought it to my attention.
