Maureen, I have looked into your account with the rebate center. They have received all five of your rebate forms. Four of the forms were received on 2.8.21 and the other was received today (3.1.21). The terms and conditions on the rebate form state that the rebate form must be submitted within 8 weeks of the order date. The dates on three of the forms you submitted were past the 8 weeks, (order date of 10.1.19, 3.1.20, and 10.10.20). The other two forms that were submitted have been processed. One rebate card was mailed out on 2.23.21 and the other that was received today is still in the processing stage and you will need to allow more time. We tried to reach out and were unsuccessful. Benita, I have reviewed your account and on 2.4.21 you placed an order for 2 boxes for your right eye. I show that you contacted us on 2.13.21 and asked to return one box for the left eye, however you did not know the correct prescription.
