Notice to employers and individuals! Do not choose this insurance plan. It’s not worth it! I have paid over $4,000 in medical expenses because this company refused to pay. Everything has been a nightmare I’ve never experienced this before. Say yourself or staff from the headache

**1 Star - 2.5 hours on hold
**1 Star — Nearly 2.5 Hours on Hold While Trying to Get Answers About Medically Necessary Surgery**
My husband and I pay approximately $950 per month for a LifeX Copay PPO health plan. Benefit Health Plan is listed as a contact for coverage verification, claims, and precertification. Unfortunately, obtaining assistance has been an exhausting and frustrating experience.
When we purchased our coverage, the plan used the Cigna network. In March 2026, the network changed to First Health. We received written assurances that providers and facilities previously in-network with Cigna would continue to have their claims processed at in-network benefit levels.
That promise has not been reflected in my experience.
I am scheduled for medically necessary surgery at Baylor University Medical Center in Dallas. Baylor was in-network under Cigna, and I received two estimates indicating my financial responsibility for hospital facility fees would be approximately $2,500–$3,141.
After the network change, Baylor classified the facility as out-of-network and increased its estimate of my financial responsibility to **$33,284.36**.
I've spent countless hours communicating with healthcare providers and the companies involved in administering this plan, trying to resolve conflicting coverage information and obtain the necessary authorizations.
**Today alone, I have spent nearly 2.5 hours on hold with Benefit Health Plan attempting to get assistance.**
This is not a minor billing question. My surgery is scheduled for October 21, and the uncertainty surrounding my coverage threatens to delay medically necessary care while potentially exposing us to tens of thousands of dollars in unexpected expenses.
I understand that multiple organizations are involved in administering this coverage, but patients should not have to spend hours on hold trying to determine whether their health insurance will function as promised.
As of this review, my situation remains unresolved.
Consumers considering coverage involving Benefit Health Plan should carefully investigate its customer service accessibility, authorization procedures, and how coverage disputes are handled. Based on my experience, I cannot recommend this company.

If I could give less than one star I…
If I could give less than one star I would. We were sold this plan through a broker as a way to save money on premiums. We have over 12 claims with them and there has been an issue with all of them. Only two claims have been paid so far and I have had to make multiple calls on those since our doctors didn’t receive detailed EOB’s to apply the payment correctly. I’m still showing balances due for well care visits that are supposed to be 100% covered. They are taking over 120 days to pay claims. 60 days to process the claims and then another full 60 days to actually mail out the checks. So we still have about ten claims that are showing outstanding since July. I also got a pre authorization for a sleep study for my son it was approved and the day before the appointment the pre authorization suddenly disappeared and benefit health plan was saying there was no pre authorization for the study. So if I would have gone through with the study they would not have paid anything of that claim. Now I am stuck trying to find an insurance we can afford and I have to wait to do the study when we have legitimate health insurance coverage. Run far far away from this company benefit health plan or lifex.

Very frustrating working with this…
Very frustrating working with this insurer. I have been trying for a month to be seen. All providers listed on the website said I was "out of network" and I could not be seen and be covered. Every phone call I made, and I made several; BHP and PHCS....everyone just pointed the finger at someone else and couldn't or wouldn't help me. I will be cancelling soon.

Benefit Health Plan did not pay almost…
Benefit Health Plan did not pay almost $2000 in invoices despite their partner LifeX Research partner company receiving almost $4,000 in premiums over a 3 month period. The Benefit Health Plan claimed that all invoices had not been paid within 30 days but proof of payment shows otherwise.
